Please note that approval or disapproval of any wholesale account shall be in Mud Pie’s sole discretion. As part of the wholesale account application, we ask for information such as the company name, “doing business as” designation, business type, full name of the applicant, email address of the applicant, Tax ID/Employer Identification Number for your business, url or website address for your business, and number or years in business.  In addition, we ask for your billing and shipping information, including billing and shipping contact name, phone number, billing and shipping address and zip code.
